HCC Coleman College for Health Sciences
Distinguished Alumni Award 2013
The Houston Community College Alumni Association (HCCAA) and HCC Coleman College for Health Sciences are proud of the accomplishments of HCC alumni in the health care industry. The HCCAA and Coleman College will recognize one distinguished alumnus who has made an impact in his or her field. Alumni, students, faculty, and staff will be invited to celebrate with the 2013 Distinguished Alumni Award recipient on Thursday, May 9, 2013 at HCC Coleman College for Health Sciences’ pinning ceremony. The award recipient will receive a certificate and an engraved gift from the College to commemorate the occasion. Additional details will be forthcoming.

Eligibility
Nominees must have graduated and hold either a certificate or associate’s degree in a health sciences field from Houston Community College and have at least five years of working experience in their field. Nominees cannot be past award recipients.
Nominators
Nominators may be any person or group with direct knowledge of the nominee’s achievements in the health care field. A nominator may nominate only one person. A person cannot self-nominate.
Selection of Award Recipients
A selection committee including representatives from Coleman College faculty and staff, HCC alumni, and HCC Alumni Association staff will review the applications and recommend the top candidate in April 2013. The recommended candidate will require final approval by the President of Coleman College and the HCC Coordinator of Alumni Relations.
Nomination Deadline
All nomination forms (see below) must be received by 2:00 p.m. on Friday, April 19, 2013. Any forms received after the deadline will not be considered.
